Plain about what we cannot do
We are not attorneys. When a matter needs legal advice or courtroom representation, we say so and point you to one.
Straight answers, before you ask
No. Entitled Document Services is not a law firm and we are not attorneys. Your forms are prepared by a certified legal document preparer, certified and regulated by a state supreme court; the About page explains exactly what that certification is. We prepare the forms you select and give general information, and we are always plain about the difference.
No. We cannot tell you which document to file, whether you qualify, or what strategy to use. Those questions call for a licensed attorney, and we are glad to suggest where to find one. What we do is prepare your chosen documents accurately, completely, and on time.
No. Only a licensed attorney or a U.S. Department of Justice accredited representative may give immigration legal advice or represent you before USCIS. You select the filings. We complete the forms using the information and decisions you provide. No somos abogados y no podemos dar asesoría legal.
Every form is a flat fee, published on this site and quoted in writing before you commit. USCIS filing fees are separate, paid to USCIS, and listed on the USCIS fees page.
Those are government fees, not ours. We tell you the exact amount, who it is paid to, and how, before anything is filed. They are never hidden inside our price.
Clients in every state. USCIS forms are federal and the same everywhere, and everything happens in writing: request, questionnaire, secure upload, review, and delivery.
Most forms are ready within a few business days. Immigration filings are not offered as rush work, because USCIS controls its own timelines; your written quote states the expected turnaround.
The version filed with USCIS must be in English, as USCIS requires. You are never signing something you have not read: we go over every page with you in writing, in Spanish, before you sign.
Need legal advice or an immigration attorney?
Some cases need more than documents. When yours does, we say so. The American Immigration Lawyers Association keeps a free search of licensed immigration attorneys by location and language, in English and Spanish.

Meet Sammijo Johnson, MLS, M.S.
Sammijo Johnson, MLS, M.S., is the founder of Entitled Document Services, LLC and an Arizona Certified Legal Document Preparer, AZCLDP No. 82291. She built the company on a simple premise: that meaningful help with legal paperwork should be accessible, transparent, and deeply attentive to the individual circumstances of every client.
Sammijo has called Arizona home for more than fifteen years. She has been happily married for more than six years and is the mother of three little girls, ages three, five, and seven, and she is a three-time Sun Devil, having earned three degrees from Arizona State University. Her work is grounded in the belief that the court and immigration systems are hard enough to navigate without the paperwork being the part that defeats you.
Her preparation for this work has been deliberate. She holds a Master of Legal Studies from the Sandra Day O'Connor College of Law at Arizona State University, with training in legal analysis, contracts, legal writing, and research, and she is pursuing a Doctor of Law and Policy at Northeastern University. Her doctoral research examines access to justice and why so many families in civil and family court end up facing the system without help at all. That research is the reason this company exists.
She prepares every document with precision and genuine care, and she is always clear about what a document preparer can and cannot do. When a matter calls for legal advice or courtroom representation, she says so and points clients toward a licensed attorney.

Her services are limited to document preparation and general legal and factual information under Arizona Code of Judicial Administration § 7-208, communications with her are not protected by attorney-client privilege, and no attorney-client relationship is created.

Legal disclaimer
Important information about the services offered through this website and the scope of what a document preparer may do.
Last updated September 5, 2026.
Professional credentials
Entitled Document Services is the trade name of Entitled Document Services, LLC, an Arizona limited liability company (ACC Business ID 25108078). Document preparation services are provided by Sammijo Johnson, an Arizona Certified Legal Document Preparer certified and regulated by the Arizona Supreme Court under Arizona Code of Judicial Administration section 7-208.
Not an attorney
Sammijo Johnson is not an attorney and is not employed by or affiliated with a lawyer or law firm. Communications with a legal document preparer are not protected by the attorney-client privilege. She does not represent that she is an attorney, hold herself out as an attorney, or provide legal services outside the scope of her Certified Legal Document Preparer credential. For matters that require legal advice or courtroom representation, clients are referred to a licensed Arizona attorney.
No attorney-client relationship
Use of this website does not create a professional-client relationship between you and Entitled Document Services, LLC. Submitting a contact form, sending an email, or requesting a service does not, by itself, create an engagement. A professional relationship begins only through a signed written agreement that describes the services and the fee.
Communications with a legal document preparer are not protected by the attorney-client privilege. Information you share may be subject to disclosure if ordered by a court. We treat your information with discretion as a professional matter, but it does not carry the legal protections that apply to communications with an attorney.
Information, not legal advice
The content on this website is provided for general informational purposes only. Nothing on this website is legal advice, and it should not be relied upon as a substitute for individualized counsel from a qualified attorney regarding your specific situation. A document preparer may provide general legal and factual information and prepare the documents you select, but may not advise you on your rights, options, defenses, or strategy, or tell you which documents to file or how to answer the questions on your forms.
Laws and court procedures change frequently. The information on this website may not reflect the most current Arizona law or your particular circumstances. Always confirm requirements with the court or a qualified professional before taking action that may affect your rights.
Immigration
Immigration document preparation is offered nationwide as the completion of USCIS forms from the information and decisions you provide. We are not attorneys and we are not accredited representatives of a recognized organization. We do not select forms, assess eligibility, advise on immigration options or consequences, interpret form questions, or represent you before USCIS, the immigration court, or any agency. Only a licensed attorney or a United States Department of Justice accredited representative may give immigration legal advice or represent you.
No guarantee of results
Every matter is unique. Outcomes depend on the facts, applicable law, the conduct of other parties, judicial discretion, and other factors outside the control of any document preparer. Nothing on this website guarantees or predicts a particular result.
Fees and costs
Preparation fees shown on this website do not include USCIS filing or biometrics fees. Those are set by USCIS and are paid by you directly to USCIS. Our rates and charges are disclosed and itemized in writing before any work begins, and current USCIS amounts are published on our USCIS fees page.
What a certified legal document preparer may and may not do
Under Arizona Code of Judicial Administration § 7-208, a certified legal document preparer may prepare legal documents for people who are not represented by an attorney, provide general legal and factual information, make forms available, and file, record, and arrange service of documents. A preparer may not give specific legal advice, opinions, or strategy; may not represent anyone in court or before an agency; may not attend court to assist a party unless permitted by law; and, with narrow statutory exceptions, may not sign the documents she prepares. Every document we prepare carries the preparer's name, the title Arizona Certified Legal Document Preparer, certificate number AZCLDP No. 82291, and the business entity name, as the rules require.
Competence and timeliness
We accept only assignments we are competent to complete accurately and on time, and we decline those we are not. We make a good-faith effort to meet promised delivery dates and notify you promptly of any delay. We do not charge contingent fees, accept anything of value tied to the outcome of a matter, or pay referral commissions.
Complaints
Certified legal document preparers are regulated by the Arizona Supreme Court. Complaints may be directed to the Certification and Licensing Division, Arizona Supreme Court, 1501 W. Washington St., Suite 104, Phoenix, AZ 85007, or through azcourts.gov/cld. Nothing in our agreements or on this website limits that right.

Terms of service
The terms that apply when you use this website and our document preparation services.
Last updated September 5, 2026.
Use of this website
This website is provided for general information about our services. By using it, you agree to these terms. If you do not agree, please do not use the website.
No legal advice, no professional relationship
Sammijo Johnson is not a lawyer, is not employed by or affiliated with a lawyer or law firm, cannot give legal advice, and communications with her are not privileged. Entitled Document Services, LLC is not a law firm and does not provide legal advice. Using this website or contacting us does not create a professional-client relationship. A relationship begins only through a signed written agreement describing the scope of services and the fee.
Your responsibilities
You are responsible for the accuracy of the information you provide and for the decisions in your matter, including which documents to file and how to answer the questions on your forms. We prepare documents based on the information and decisions you give us.
Fees and payment
Preparation fees are flat fees, published on our forms menu and disclosed and itemized in writing before work begins. USCIS filing and biometrics fees are set by USCIS, are never part of our fee, and are paid by you directly to USCIS when you file, by check or money order, by card with Form G-1450, or online through your USCIS account. Current amounts and official links are listed on our USCIS fees page. Work begins after your agreement is signed and payment is received.
Filing
You file your own forms with USCIS. We deliver the completed forms with a checklist and plain-language filing instructions, and we sign the preparer block on every form we prepare. We do not represent you before USCIS or any other agency and do not submit filings under our own name or account.
Turnaround
Rush preparation is not offered on this site. Your written quote states the expected turnaround for your forms, and USCIS processing times are set by USCIS and cannot be rushed by us.
Cancellations and refunds
You may cancel for a full refund by written notice to us by email or WhatsApp message within 24 hours after your payment is received, or within 48 hours for orders that include more than one service. The window is measured from the time we receive your payment. After it closes, fees are non-refundable. USCIS fees are paid by you directly to USCIS and are not refundable by us. If USCIS rejects a form because of a preparation or formatting error we made, we correct and re-prepare it at no charge; that correction is your remedy for preparation errors.
Billing questions, chargebacks, and disputes
If you have a question about any charge, contact us first in writing at billing@entitleddocs.com or by WhatsApp and allow one business day for a response; most billing questions are resolved the same day. By paying, you acknowledge that you are the authorized cardholder, that our services are delivered electronically to the email address you provide, that the refund terms were disclosed to you before payment, and that you will not initiate a chargeback for services delivered as described. A chargeback filed contrary to these terms is a breach of your agreement and may be pursued as a civil claim for the amount reversed, our dispute fees, and our costs and reasonable attorney fees where authorized by A.R.S. § 12-341.01, in small claims court or another court of competent jurisdiction. Before filing any claim, dispute, or chargeback, you agree to give us written notice and five business days to correct the concern. Disputes we cannot resolve informally are resolved by binding individual arbitration under the American Arbitration Association Consumer Arbitration Rules in Maricopa County, Arizona, except that either party may bring an individual claim in small claims court. Nothing in these terms limits your right to file a complaint with the Certification and Licensing Division of the Arizona Supreme Court.
Confidentiality and conflicts of interest
We treat the information you provide as confidential and use it only to prepare your documents, subject to disclosure required by law. Communications with a legal document preparer are not protected by the attorney-client privilege. If an actual or apparent conflict of interest arises in your matter, we will disclose it to you in writing and will not provide or continue services unless you waive it in a writing signed by both parties.
Electronic signatures and records
Agreements are signed electronically. Your electronic signature has the same effect as a handwritten signature under the Arizona Electronic Transactions Act (A.R.S. § 44-7001 et seq.) and the federal ESIGN Act. A complete copy of each signed agreement, including the agreement text, your typed and drawn signatures, and the signing timestamp, is kept on file and provided to you.

Sign your document preparation agreement
Our certification rules require a written agreement between you and your document preparer before work begins. Read it, fill in the details from your written quote, and sign below.
Document Preparation Services Agreement
Parties. This agreement is between Entitled Document Services, LLC ("we," "us") and the client named below ("you"). Document preparation services are provided by Sammijo Johnson, an Arizona Certified Legal Document Preparer, AZCLDP No. 82291, certified and regulated by the Arizona Supreme Court under Arizona Code of Judicial Administration § 7-208.
Not an attorney; no legal advice. Sammijo Johnson is not an attorney, is not employed by or affiliated with a lawyer or law firm, and is not licensed to practice law in Arizona or any other jurisdiction. We prepare the documents you select and may provide general legal and factual information, but we cannot give legal advice, tell you which documents to file, advise you on your rights, options, or strategy, or represent you in court or before any agency. If you need legal advice or representation, you should consult a licensed attorney. Communications with a legal document preparer are not protected by the attorney-client privilege.
Services. We will prepare the documents described below, based on the information and decisions you provide, and deliver them with filing instructions and a completeness checklist. You are responsible for the accuracy of the information you give us and for reviewing every document before it is signed or filed.
Confidentiality. We treat the information and documents you provide as confidential and use them only to prepare your documents, subject to any disclosure required by law. As stated above, communications with a legal document preparer are not protected by the attorney-client privilege.
Fees. Our preparation fee is the flat fee stated below and confirmed in your written quote, which discloses and itemizes our rates and charges in writing before any work begins. USCIS filing and biometrics fees are set by USCIS, are not included, and are paid by you directly to USCIS when you file. If USCIS rejects a form we prepared because of a preparation or formatting error, we correct and re-prepare it at no charge.
Filing. We prepare your forms and you file them with USCIS using the written instructions and checklist we deliver with them. We sign the preparer declaration on every form we prepare. We do not represent you before USCIS or any other agency and do not submit filings under our own name or account.
Payment, cancellation, and refunds. Fees are due as stated in your written quote, and work begins once this agreement is signed and payment or any required deposit is received. You may cancel for a full refund by written notice to us by email or WhatsApp message within 24 hours after your payment is received; for orders that include more than one service, that window is 48 hours. The window is measured from the time we receive your payment. After the window closes, fees are non-refundable. USCIS fees are paid by you directly to USCIS and are not refundable by us. The correction promise above is your remedy for preparation or formatting errors.
Timing. Standard turnaround is stated in your quote. Rush service is not offered for immigration forms; USCIS processing times are set by USCIS and are separate from our turnaround.
Billing questions and chargebacks. If you have a question or concern about any charge, you agree to contact us first in writing at billing@entitleddocs.com or by WhatsApp and allow one business day for a response; most billing concerns are resolved the same day. By signing this agreement and submitting payment, you acknowledge and agree that: (1) you are the authorized holder of the payment method used and you authorize the charges described in your written quote; (2) these are custom document preparation services delivered electronically, and delivery to the email address you provide constitutes receipt; (3) the refund policy above was disclosed to you in writing before payment and you accept it; (4) you will not initiate a payment dispute or chargeback for services delivered as described in this agreement, and a chargeback made contrary to this paragraph is a breach of this agreement; and (5) if such a chargeback is filed, we may submit this signed agreement, our delivery records, and our communications with you as evidence, and you remain responsible for the fees for work performed and for any dispute fees our payment processor charges us, to the extent permitted by law. Charges appear on your card statement under our business name, Entitled Document Services.
Dispute resolution. Before filing any claim, dispute, or chargeback, you agree to give us written notice describing the concern and five business days to correct it; the correction promise above is part of that cure opportunity. Except for the small-claims right below, any dispute arising out of this agreement or our services that we cannot resolve informally will be resolved by binding individual arbitration administered by the American Arbitration Association under its Consumer Arbitration Rules, in Maricopa County, Arizona, or by phone or video, and judgment on the award may be entered in any court with jurisdiction. Either party may instead bring an individual claim in small claims court. A chargeback reversed contrary to this agreement may be pursued as a civil claim for the amount reversed, our dispute fees, and our costs and reasonable attorney fees where authorized by A.R.S. § 12-341.01 or other law, in small claims court or another court of competent jurisdiction. Claims proceed individually and not as class or representative actions, to the extent permitted by law. This agreement is governed by Arizona law. Nothing in this agreement limits your right to file a complaint with the Certification and Licensing Division of the Arizona Supreme Court.
Conflicts of interest. If an actual or apparent conflict of interest arises in your matter, we will disclose it to you in writing and will not provide or continue services unless you choose to waive the conflict in a writing signed by both parties.
Complaints. Certified legal document preparers are regulated by the Arizona Supreme Court. Complaints may be directed to the Board of Legal Document Preparers, Certification and Licensing Division, Arizona Supreme Court, 1501 W. Tell us the details, in your own words
Pick your service and answer what you can. Skip anything you are unsure about; we will go over it together. Nothing here is legal advice, and you decide which documents to file.
Send us your documents
Upload photos or PDFs of the documents for your matter. Files travel encrypted and are stored privately for our review. Up to four files per submission, 8 MB each; send another submission if you have more.
Book a call
Calls are reserved for existing clients and ongoing matters. If your documents are already underway, pick a time that suits you and we will call you to go over where things stand and what comes next. Calls are in English and are available Monday through Friday, with Saturday appointments for current clients only. If you prefer Spanish, we are happy to keep everything in writing by email or WhatsApp.
